There is a section in the Funding Information tab of the revocable trust binder that explains how to re-title separate property:
TITLING AN ASSET WHEN YOU HAVE A REVOCABLE LIVING TRUST
Ownership wording for financial assets and contingent beneficiary wording for assets with named beneficiaries (such as IRAs, 401Ks, annuities, life insurance, etc.). Note the last phrase of the vesting language when it is someone's separate property.
Examples:
Joint Property between Spouses: "JOHN DOE and JANE DONE, Co-Trustees, or their successors in Trust, of The John and Jane Doe Revocable Living Trust dated 1/1/2022."
Separate Property of JOHN DOE: "JOHN DOE and JANE DONE, Co-Trustees, or their successors in Trust, of The John and Jane Doe Revocable Living Trust dated 1/1/2022, as JOHN DOE's sole and separate property."
Separate Property of JANE DOE: "JOHN DOE and JANE DONE, Co-Trustees, or their successors in Trust, of The John and Jane Doe Revocable Living Trust dated 1/1/2022, as JANE DOE's sole and separate property."
